CarePath Terms of Use

Effective October 27, 2026

These Terms of Use ("Terms") apply to CarePath on the web and in the mobile app, provided by PairaDoc Technologies LLC, an Oklahoma limited liability company ("PairaDoc," "we," "us"). By using CarePath, you agree to these Terms. If you do not agree, please do not use CarePath.

What CarePath is

CarePath is a free educational resource about serious illness, hospital and ICU care, end-of-life questions and advance care planning. It includes educational articles, equipment guides, links to official state resources, and optional planning worksheets you can save as PDFs on your device. CarePath does not require an account, and there is no charge to use it.

Educational information only

CarePath provides general information, not medical, legal, financial or other professional advice. It does not diagnose, treat, or recommend care for any person, and using it does not create a healthcare provider–patient or attorney–client relationship. Always talk with the treating healthcare team about a specific person's care, and with a qualified attorney or official state resources about legal documents. Our full Legal & Educational Disclaimer is part of these Terms.

You are responsible for reviewing what you prepare

CarePath worksheets and PDFs are built from what you type. You are responsible for reviewing them for accuracy before you rely on or share them. CarePath worksheets are not legally executed advance directives, healthcare powers of attorney, DNR orders, POLST or other medical orders. Official forms and their signing, witness and notarization requirements must be completed separately.

Not for emergencies

CarePath is not an emergency, monitoring or crisis service. In an emergency, call your local emergency number. For urgent questions about someone's care, contact their healthcare team directly.

Who may use CarePath

CarePath is intended for adults and families. It is not directed to children under 13. If you use CarePath on behalf of someone else, you are responsible for respecting that person's privacy and wishes.

Acceptable use

Please use CarePath only for lawful, personal and educational purposes. Do not attempt to disrupt, overload, probe or gain unauthorized access to CarePath or the systems that host it, copy it in bulk with automated tools, or use it to mislead others. Do not send us patient records or sensitive health information through feedback or email.

Our content

The text, illustrations, design and other content of CarePath belong to PairaDoc Technologies LLC or its licensors. You may view, print and share CarePath pages and your own worksheets for personal, non-commercial use. Please do not republish, sell or modify CarePath content without our written permission. Some equipment illustrations are AI-generated educational images and are labeled as such.

Links to other websites

CarePath links to government agencies, nonprofits and other organizations for convenience. We do not control those websites and are not responsible for their content, accuracy, availability or privacy practices.

Privacy

Our Privacy Notice explains how CarePath handles information. In short, worksheet answers stay on your device, and CarePath has no accounts, analytics or advertising.

Changes and availability

We may update, add to, or remove CarePath content or features at any time, and CarePath may sometimes be unavailable. Information may become out of date even though we work to keep it current. If we change these Terms, we will update the effective date above. Continuing to use CarePath after a change means you accept the updated Terms.

Disclaimer of warranties

CarePath is provided "as is" and "as available," without warranties of any kind, whether express or implied, including warranties of accuracy, completeness, merchantability, fitness for a particular purpose and non-infringement, to the fullest extent permitted by law.

Limitation of liability

To the fullest extent permitted by law, PairaDoc Technologies LLC and its members, employees and contractors will not be liable for any indirect, incidental, special, consequential or punitive damages, or for any decision made or action taken based on CarePath content. Because CarePath is free, our total liability for any claim relating to CarePath is limited to fifty U.S. dollars ($50). Some jurisdictions do not allow certain limitations, so some of these may not apply to you.

Indemnification

If you misuse CarePath or violate these Terms, you agree to be responsible for resulting claims and costs, including reasonable attorneys' fees, brought against PairaDoc Technologies LLC, to the extent permitted by law.

Governing law

These Terms are governed by the laws of the State of Oklahoma, without regard to its conflict-of-law rules. Any dispute relating to CarePath will be brought in the state or federal courts located in Oklahoma County, Oklahoma, unless applicable law requires otherwise.

App store terms

If you download CarePath from the Apple App Store or Google Play, the store's own terms also apply to your download. Apple and Google are not responsible for CarePath or its content. Apple and its subsidiaries are third-party beneficiaries of these Terms for app versions downloaded from the App Store.

General

If any part of these Terms is found unenforceable, the rest remains in effect. Our not enforcing a part of these Terms is not a waiver. These Terms, the Privacy Notice and the Legal & Educational Disclaimer are the full agreement between you and us about CarePath.
