ABOUT THIS RESOURCE
Legal & Educational Disclaimer
CarePath is a free educational and resource-navigation platform provided by PairaDoc Technologies LLC. We help individuals and families learn about serious illness, hospitalization, end-of-life questions, and advance care planning. Our information and worksheets are starting points for understanding and conversations, not instructions about what an individual must do.
Education and medical care
CarePath shares general educational information. It does not provide individualized medical advice, diagnosis, treatment, or recommendations, and does not replace the professional judgment or advice of a patient's treating healthcare team. Using CarePath does not create a healthcare provider–patient relationship. Discuss medical decisions and questions about a particular person with their healthcare professionals.
Legal information and applicable law
CarePath provides general legal education and links to resources, not legal advice or a legal document preparation service. Using this platform does not create an attorney–client relationship. Nothing on CarePath replaces, changes, or overrides any applicable state or federal law, regulation, legal document execution requirement, or medical-order procedure. Applicable laws and official requirements take precedence over summaries on this site.
Advance directives and official forms
Requirements differ among jurisdictions and document types. They may include particular language, eligible witnesses, notarization, signatures, healthcare agent selection or restrictions, and authorization by healthcare professionals. Verify the current requirements for the specific document and location, using official state resources or a qualified professional before relying on a document as legally effective.
CarePath educational worksheets
CarePath's planning worksheets can help you organize your wishes and share preferences with people you trust. They are not legally executed advance directives, healthcare powers of attorney, do-not-resuscitate (DNR) orders, POLST or similar medical-order forms, or other binding medical or legal instruments. Downloading, printing, signing, or sharing a CarePath worksheet does not automatically create a legally valid document or establish legal decision-making authority. Official forms and required execution steps must be handled separately.
Explore the planning worksheets or visit the State Legal Navigator to find starting points for discussion and official resources.
Information, links, and changing requirements
Information can change, and general explanations may not apply to every person's situation. CarePath cannot guarantee that information is always complete or current or that any particular medical or legal result will occur. Links to government agencies, nonprofits, and other organizations are offered for convenience and further research. CarePath does not control, endorse every statement on, or guarantee the accuracy or continued availability of external websites.
Emergencies and immediate concerns
CarePath is not an emergency, monitoring, or crisis-response service. For emergencies, contact your local emergency services. For urgent questions about care in a hospital or other setting, contact the treating healthcare team directly.
Privacy and sharing
CarePath's worksheet answers are handled locally in your device's open page. PDF creation and Share My Wishes use local files and device-based sharing; CarePath does not receive the worksheet contents or the contact details of your chosen recipients. Other services involved in saving or sharing may handle information according to their own policies. Hosting providers may also process necessary technical, network, and security logs. For the complete and current description of these practices, read our separate CarePath Privacy Notice.
Questions or corrections
If you find an unclear explanation or an outdated source, you can suggest a correction or resource. Avoid including private patient information in feedback.
