No account required. CarePath does not save this form or upload your entries; it prepares an email in your own email app for you to review and send.
Do not include names of patients, diagnoses, medical records, personal contact details, or other sensitive health information. Your email provider and CarePath's support mailbox will handle any email you choose to send under their respective policies.
Submissions are reviewed before any changes are made. Sending a suggestion does not automatically add a resource to CarePath.
What happens to suggestions?
We review suggested links for relevance, accuracy, accessibility, and source authority. For state-specific legal information, we prioritize official statutes and government-issued forms. Resources are not automatically published, and we cannot respond to medical emergencies or provide individualized legal advice.
